Parents, complete this survey by the first day of school and your child will not have any homework for me the first week of school. :D So, you can essentially do your child's homework. Please make sure you put your child's name so I can give them credit.

Thank you in advance for completing this survey. The information you provide will better help me prepare to teach your child this year. If you have suggestions or concerns that were not addressed in this survey, please write them down and send them to me or send me an e-mail. I welcome your input.   1. The projected cost of this year's field trip is $40 per child. We will again sell coupon books to help pay for the trip. Many students covered the entire cost of their trip by selling coupon books. To fully pay for the trip, students will need to sell 2 packets of 5 books (or 10 books). The top seller will receive a prize. Do you plan to participate in the fundraiser this year? Please choose one of the following.*
    I do not plan to partcipate. We will pay the $40 in full one month before the trip.

  1. Last year I started putting some quizzes and tests on Quia. Most students really liked this option because they could see their scores right away on the computer screen along with the correct answer (if they missed something). How do you feel about tests/quizzes on the computer? (strongly agree= like computer test and strongly disagree= prefer paper and pencil test)*

  1. This year I am offering an on-line homework option on a trial basis. I will provide Quia games your child may choose to do instead of the homework sheet. Your child may play the games as many times as he or she likes. I will put the highest grade earned into the gradebook on Friday, so your child will have from Monday until Thursday to complete the homework assignment. For students who do not choose this option, the homework sheet will be due at the beginning of class on Friday. Would this option help your child with homework and understanding the material better?*
