Thank you for attending or for signing up for the Hoops 101 Select 2011 tryout at the Dripping Springs YMCA. As promised, this is a questionnaire to get some more information about each of you.

Our coaches will process these responses as we try to determine each player's mindset and mentality, along with their skill and competitive levels.
