Hoops 101 Select Parent Midseason Report Card
In an effort to get your feedback ahead of time so that we can be sure to be efficient with time and address any and all concerns you may have, we have created the following survey. It will only take a couple of minutes. Please be honest and open. We want a real discussion based on your real concerns and real opinions.

- How would you rate your team's commitment level to Hoops 101 Select (practice attendance, academy attendance, participation in tournaments, etc)*
