JCC Foundation Board Questionnaire 2012
The questionnaire items below are part of the Functional Area Review process currently underway at the Foundation. Please select the response for each item which most closely represents your opinions. Please answer all questions. Comments are optional, but are invited. A summary of total results will be discussed during our April Board meeting. Thank you very much for participating in the Foundation’s review process. Please note: When you are finished completing your survey, click on the submit button. You will receive a message indicating your answers were submitted successfully. If you do not receive that message, your survey will not be received. Although we are asking you to input your name, this will only be used to contact you to clarify the responses you've submitted. Your answers will be kept confidential, and only the person compiling the survey results will see your name. Please submit your responses no later than Wednesday, February 8th.
