LPI (Leadership Practices Inventory) - Name of Leader - Insert Name - (copy)
Welcome to the LPI and thank you for agreeing to take part. There are 35 questions in this survey. The first asks you to identify whether you are the leader, leader's manager, direct report, co-worker or other. Questions 2-31 ask 'To what extent does the leader typically engage in the following behaviours? You should choose the response number that best applies to each statement. The rating scale runs from 1 to 10 - 1 = Almost Never 2 = Rarely 3 = Seldom 4 = Once in a while 5 = Occasionally 6 = Sometimes 7 = Fairly Often 8 = Usually 9 = Very Frequently 10 = Almost Always Questions 31-35 are free response questions. These are not compulsory but provide very useful information to the leader. (If you are the leader, please leave blank).
