Transportation and Respiratory Symptoms

The overall purpose of this project is to determine the use of public transportation (buses) on respiratory infection in high school students. You are being asked these survey questions in order to determine the effect of different variables on respiratory health. This survey is only meant for students of the ages 13 to 18. Thank you for your participation.

In order to participate, you must verify that you are of sound mind and judgment and can honestly consent to experimentation. Participation requires that you be subjected to questions about your transportation use and health online via this survey. This entails answering the following questions with full honesty and with a great deal of sincerity. If a participant for any reason chooses to withdraw consent at any time, there will be no penalization or consequence. There are no foreseeable medical risks, but should anything occur during testing, contact the hospital, police, etc. for any necessary assistance. Benefits of participation are aid to the epidemiological community in the field of respiratory health and the affect public transportation use has on it. If you desire a completed report of the investigation, it may be requested directly from the student researcher.
