Computer Use Survey 2012-13

Please take your time to answer the following questions. No music or other open websites while you take the survey please.

Those that have an area for your to TYPE a response, please be detailed and honest. The more information you give me the better I can help you. Notice that the questions that require you to type information have a LARGE area given for you to do so. That means you should TYPE a LOT of information.
