Decision Tree Feedback Form - 10/24/12 - DW

Use this survey to submit issues that you encounter with D-Tree. Copy and Paste as much information as you can, and if asked, leave a brief description of the exact issue. It is important that you submit this information as it comes in. If you do not submit it, we will not be able to collect the data. We will attempt to replicate all issues submitted before sending to Samsung for resolution. Agents who wish to receive credit should fill out their full name. Do not wait to submit issues. You may submit as many as you find. Best practice is to submit the issue you encountered at the time of the encounter.

  1. Copy and Paste Model Code for models that have no Decision Tree information. Then submit the survey:


  1. Copy and Paste the Model Code and path you took to attempt to find information that was not found. Leave a brief description about what you were looking for that was not found. Ex: No How To Guide, or No Service Manual, No Troubleshooting for this issue...


  1. Sap-GCIC Errors: Model Code, Serial # and description of issue when performing action. Ex: Set up of Exchange that requires purchase date, but no date can be entered (also include transaction #)


  1. Others: Any other issues you have with D-Tree can be placed here, keep in mind we need to be able to replicate these problems before submitting them to Samsung to be fixed.
