Mid Year Review FY 2005 Survey

In April and May of each year, we typically review employee performance for the first six months of each fiscal year. Please help us improve our review process.

Please answer candidly - all responses are anonymous and cannot be connected to any individual.

These questions apply to staff who were in their jobs full time as of 2/1/05. If you started after that date, you would most likely not have been reviewed.
