Burke's Fall 2014 After-School Sports Availability Form and Sign-Up

If your daughter is signing up to join one of the Burke’s athletics teams this fall please fill out the schedule below with complete and accurate information about her availability. If there are days that she needs to leave early or has an unusual time pressure please include that information as well. Please complete this form as soon as possible. The final deadline is Monday September 1st, which will allow us to plan try-outs accordingly and schedule the season.

If your daughter has a flexible schedule or can rearrange other activities around her commitment to her team please indicate this by including all possible days she could practice and play. Her coach will let her know the days that work best for her team.

  1. If you are a fifth or sixth grade athlete and are planning to try-out for a spot on a league Volleyball team please answer this question. If you do not make a spot on a league Volleyball roster should we transfer your information to the 5th or 6th grade volleyball roster instead? Answering "No" to this question will not improve your chances of making a league team. However, we will take a "yes" answer to be a commitment to your the Development team for your grade level.

  1. Please indicate below the days on which you are free for participation in school sports this Fall. Remember for Cross-Country or League Volleyball you must have at least three regular practice days a week (Mon - Fri), which is the minimum time commitment needed to play on one of our League teams. For 5th or 6th grade volleyball you must have at least two regular practice days (Mon - Thurs). Practices and games will usually happen between 3:30 and 6pm. If you could be free on more than three days please check them all and depending on which team you make we can decide what will be the best schedule for you.

  1. If there are specific conflicts or time pressures that might affect her being able to practice on certain days please outline the details below. The more information we have the better able we are to try to serve the needs of all athletes.


  1. League Volleyball teams may have practices scheduled from 7am to 8am to allow sufficient time and space for meaningful practice together as a team, and to free up some afternoon time on your schedule to meet with teachers and do other things you need to do. Please initial below to indicate that you understand that your daughter will need to make every effort to attend morning practices if her team has them scheduled. This question does not apply to 5th or 6th grade volleyball or cross-country.


  1. Players are placed on teams based on the way we feel their level of play best matches the level of competition. We will determine a specific practice schedule for each team once the teams have been formed. Your daughter will be expected to attend all of her team’s practices on the days that you have indicated that she is regularly available. She will also be expected to be available for ALL of her team’s games for League teams and all morning practices. Should a change arise in her schedule, please provide us with updated information in writing as soon as possible.

    If your daughter is ill or unable for special circumstances to attend a practice or game, prior notice must be given to the coach.

    •Coaches may choose to limit playing time for an athlete who has been absent without permission or notification.
    •Persistent and / or unexcused absences may result in an athlete’s discharge from the team.

    Please be sure that your daughter brings her sports uniform on game days. She must wear a full Burke's uniform to play in our games.

  1. The BAIAL and SFAL schools subscribe to the California Interscholastic Federation guidelines for adult spectator conduct at school games.
