Thank you for participating in the selection of the Q3 2014 Ovation! Award winners. Associates recognized demonstrate significant and sustained performance that has a measurable impact to Comdata and/or the customer (cost savings, revenue impact, process improvement, etc.).



Survey Instructions: 8 nominations are being submitted for the Q3 2014 Ovation! Award. Names of the nominees have been omitted to ensure selection based on action and characteristics for which one is nominated.

(1): Individual nominated twice - Nomination #1: _____ worked above and beyond supporting all efforts to thwart fraud arising from the Home Depot data compromise. He worked extra hours, he covered for other employees and he provided recommendations and insights to make the process more efficient and customer focus. His actions helped Comdata avoid over $90K in increase fraud incidences in the past two weeks" Nomination #2: We are short staffed for 1st shift in our department right now. He has always been hard-working and reliable, but here lately, he has even surpassed his own self; practically working all by himself last week. This week, he has been working SO hard at the latest outside data breach that is affecting our customers. _____ always works honestly, never calls out sick, always does the right thing when working cases and follows company policy and procedures consistently. He is 100% focused on helping our customers who are victimized by fraud, and goes above and beyond to help customers with their case, even if he doesn't have to, or it's not part of his job. He takes his time with the customers, while also working efficiently to investigate and close as many fraud cases as possible. He also works hard to find additional fraud that our systems may have not yet identified, which is helping the customers who have no idea they have been compromised. This instills loyalty and value to our customers. These skills combined are difficult to find in most people, but he embodies this, 100%. This week, he has been devoting his time and skills to identifying as many customers of ours as possible that have been affected by a recent outside data breach.



(2):_____ continues to use his talent and skills to improve processes and develop programs which assist in account setup for Fleet ITS, but also extends beyond implementations out to various departments company wide. _____ is repeatedly called upon to assist in critical time sensitive situations where an update, a change or flagging must take place across the masses. Heaccepts this willingly. _____ assisted Risk Management with the Home Depot breach with card information that saved the risk department hours of manual work ($2500+ in labor alone). He has written and processed macros to move our Emdeon accounts over to the full funding model, saving many customers that were on the brink of leaving due to inability to reconcile accounts. _____ assisted with Tango to set the Vendor Surcharge fields appropriately, significantly reducing manual effort He has written a program to quickly set up Comdata Direct vendors reducing the set up time by min 3 days. He is a team player, great asset to Comdata. He works all hours, often is called upon at the last minute, and even while on PDO. He takes the meaning of team player to a whole new level, and with a great attitude.

(4): _____ was asked to step up and lead the newly formed Prepaid Open Loop Application Development team. The announcement went out, and _____ assumed his new role on July 1st, 2014. On July 2nd, 2014, and again on August 1st, 2014 the Open Loop platform suffered significant issues causing our clients and card holders to not receive funds as expected. Since a large number of our customers use the product as a Payroll card, the lack of timely funds has a significant impact on their lives. Card holders and clients had lost faith in the Prepaid Open Loop product. _____ immediately dug in, took his team, set up a "War Room" and proceeded over the next six weeks to both ensure production ran successfully as well as implemented change after change to improve stability and performance. _____'s dedication, determination and leadership were critical to the success the team has experienced as we work to restore client confidence. He worked across the organization pulling in resources as appropriate while walking the balance between the urgent need for improvement and the cautionary approach of doing no harm.



(5): Individual nominated twice - Nomination #1:_____ stands in a class all by herself when it comes to being proactive in coming up with new revenue opportunities. Not only does she come up with the idea, she researches the market, she pulls together all of the customer data, she puts together a plan to implement (HIGH PERFORMING), then she works with Marketing, Legal, Product, Sales and RCS Operations to get internal and external communications sent out (TEAMWORK). In the past quarter, she has identified opportunities to renew licenses for customers impacted by new regulations ($3K), she identified 400+ customers that have not used us in the past 12 months and suggested a discounted rate for 90 days to entice them to utilize us again. She found we were losing a potential $20K from customers that had contracts older than 2009 and were not being charged the appropriate annual permit rate, as well as researched a number of NEW permits being issued by states that we didn't have in our system. We are still tracking the revenue we are beginning to generate as a result of these new permit types. Thus far, _____ is responsible for at least $10K in new revenue based on her efforts this past quarter!!! Priceless! Nomination #2: _____ provides our mutual clients with that "wow" factor by always managing to be proactive in disseminating vital information, providing updates to our processes, and leading the charge on just "troubleshooting" in general.She is the most self-motivated Associate I've observed, and never misses the opportunity to bring a great idea to the Sales team's attention to either improve the overall customer experience, drive new revenue, or uncover new RCS prospects. _____ is a pleasure to work with and her unbridled passion for "doing the right thing" for our customers, and for the good-of-the-order make her an outstanding candidate for this award.



(6): Simply put, _____ is great. She, on numerous occasions, has gone out of her way to help with me fuel studies, specific pricing requests or just random questions from prospective clients about discounts. She is the type of person who will drop what she is doing to help to offer her support or assistance. Specifically, _____ assisted me with some fuel data and was creative in how she put it together for the prospective client. Ultimately, her efforts contributed to us earning the business of a 2,000 vehicle fleet, that spends over $1.5M/month on fuel. She's reliable, positive, accountable and creative. Her actions speak to her genuine desire to help our clients and our sales teams. She brings a lot of experience and great ideas to help us win new deals and help our clients reduce their fuel costs. She is a valuable asset to our business and a great teammate to have in the fleet business.



(7): _____ has been integral in onboarding and growing the Pay For You service line of business and growing our relationship with who is now probably our largest customer Tangoe. She trained Comdata team members internally, delivered training via WebEx to a small group of agents at West (our outsource partner) and traveled to Guadalajara Mexico to train a group of thirty agents at a West site. ________ holds daily meetings with the client to address implementation and process issues. She takes action items and delivers at almost every meeting. She addresses client concerns and corrects agent behaviors that are affecting the relationship with the client. _____ coordinates efforts and always keeps efficiency and scalability in mind when updating processes and procedures which is crucial when managing manual processes. When Tangoe made the decision to allow their team members to enroll vendors onto our platform, _____ traveled to Tangoe sites to deliver the training. _____ has become a key member of this project and a respected business partner to the client. She has gone above and stepped clearly out of her role to ensure that we win Tangoe's confidence. The results are astonishing. Prior to acquiring the Tangoe business, our Pay For You team processed on average 10MM in spend a month. Since on boarding this client our processing has jumped to over 60MM in spend by month end in September. We could not have done this without her and her leadership.



(8): From July 1 - September 2, 2014, _____ was invaluable to our department. This was the IRS 2290 filing season and we received thousands of phone calls and emails from customers which were having issues filing their 2290. If _____ had not been there to help us assist these customers, we would have had many upset customers not wanting to use our service anymore. Each time someone from our department contacted _____ either by phone or email, he was always willing to help no matter what else he had going on. He was an invaluable part of our team and always exhibited a customer driven goal of making sure all issues were handled in a timely manner and resolved to the customer's satisfaction.
