To complete your Field Placement Hours, please choose from the dates and times below. If you have 8 hours to complete, please choose 2 dates (mark 2 boxes). Each day consists of 5 hours which will include a training at the beginning of the day. Students will be chosen on a first come first served basis and we recommend that you choose your dates as soon as possible. Please email Alexa Diaz at alexa@jasouthflorida.org if you have additional questions. You will receive an email from a JA staff member to confirm your dates after you submit this form. PLEASE WAIT 1-2 BUSINESS DAYS FOR THIS CONFIRMATION VIA EMAIL. Please bring your Rubric and Clearance Badge on the day of your visit. It is a requirement in order to volunteer. CANCELLATION POLICY If you are unable to attend on the date that you have registered for or need to make a change, we require you to do so with at least 48 hours in advance. If you are sick, we ask you to provide a doctor’s note. We understand that situations might arise but want to remind you that your presence is necessary for our program to be successful.
