Self Understanding: Self Awareness of skills and limitations. Morris can accurately and independently assess and describe his skills and limitations related to academic goals.
1 - Poor: Cannot describe his skills or limitations as related to possible academic goals.
2 - Fair: May struggle with describing either skills or limitations, and has the ability to describe one or the other, with guidance.
3 - Good: Can describe both skills and limitations with guidance or teacher support.
4 - Excellent: Can accurately and independently describe his skills and limitations related to academic goals.
Self-Advocacy/Assertiveness: Asks for help or direction when needed.
1 - Poor: Does not ask for help or direction
2 - Fair: Asks for help or direction only after being prompted to by teacher.
3 - Good: Inconsistently asks for help or direction independently when assignments or concepts are unclear.
4 - Excellent: Student consistently and independently asks for help or directions when assignments or concepts are unclear.
Accommodations: Requests all accommodations appropriately and independently.
1 - Poor: Special Education teacher assists student in informing instructors of student's need for accommodations.
2 - Fair: Requests accommodations in some classes, with guidance and support from Special Education teacher.
3 - Good: Requests accommodations appropriately, but with occasional special education support.
4 - Excellent: Requests all accommodations appropriately and independently.
Communication: Communicate with teachers to clarify expectations.
1 - Poor: Student rarely approaches teachers to clarify expectations.
2 - Occasionally approaches teachers to clarify expectations.
3 - Good: Usually approaches teachers to clarify expectations.
4 - Excellent: Always approaches teachers to clarify expectations.
Work Completion: Takes extra measures to ensure work is completed and turned in 100% of the time.
1 - Poor: Does not complete work on time or at all.
2 - Fair: Often turns in work that is incomplete or late.
3 - Good: Completes work most of the time, however, work is often turned in late and/or partially complete.
4 - Excellent: Completes and turns in work on time. Promptly arranges for and completes work after an absence.
Initiation and Persistence: Morris consistently begins assignments without assistance. He follows directions and completes assignments without prompts.
1 - Poor: Does not begin assignment at all.
2 - Fair: Often begins assignment
3 - Good: Completes work most of the time, however, work is often turned in late and/or partially complete.
4 - Excellent: Completes and turns in work on time. Promptly arranges for and completes work after an absence.
