  1. Self Understanding: Self Awareness of skills and limitations. Morris can accurately and independently assess and describe his skills and limitations related to academic goals.
    1 - Poor: Cannot describe his skills or limitations as related to possible academic goals.
    2 - Fair: May struggle with describing either skills or limitations, and has the ability to describe one or the other, with guidance.
    3 - Good: Can describe both skills and limitations with guidance or teacher support.
    4 - Excellent: Can accurately and independently describe his skills and limitations related to academic goals.


  1. Self-Advocacy/Assertiveness: Asks for help or direction when needed.
    1 - Poor: Does not ask for help or direction
    2 - Fair: Asks for help or direction only after being prompted to by teacher.
    3 - Good: Inconsistently asks for help or direction independently when assignments or concepts are unclear.
    4 - Excellent: Student consistently and independently asks for help or directions when assignments or concepts are unclear.


  1. Accommodations: Requests all accommodations appropriately and independently.
    1 - Poor: Special Education teacher assists student in informing instructors of student's need for accommodations.
    2 - Fair: Requests accommodations in some classes, with guidance and support from Special Education teacher.
    3 - Good: Requests accommodations appropriately, but with occasional special education support.
    4 - Excellent: Requests all accommodations appropriately and independently.
