Post Coaching Feedback - ACT Form
Purpose: To measure overall understanding of the tenets (Quality Behaviors) discussed during coaching. When answering the questions, please focus only on the performance feedback provided to you that were based on the quality requirements detailed in the ACT form. Please avoid rating the coach. Note: Behaviors (tenets) refers to any requirement that is listed in the ACT form (ex: Professionalism, verification, documentation, audible branding etc). This survey is anonymous.
