Uniform Order-2015
Please complete this survey/order by Thursday, June 4, 2015 at 4 PM. Be sure to indicate the school. All employees will receive new shirts. Dual managers please complete a survey/order for each school. Thank you.

- List each employee's first name and the requested uniform size next to it. Size options are:
Men's: Extra Small, Small, Medium, Large, Extra Large, 1X, 2X, 3X, 4X (Mens' sizes are comparable to current uniform fit.)
Women's: Extra Small, Small, Medium, Large, Extra Large, 1X, 2X, 3X, 4X (Ladies' sizes run smaller, so order the next larger size. If you took a lady's medium, then order a lady's large.)*
