Presidential Performance Summary Form 2014-2015 Instructions: This response form is based on the President's goals for 2014-2015. These goals are attached to the e-mail with the link to this survey. A space has been left after each question for you to write comments. Please type your name where indicated on the survey (please be assured your survey will be confidential, and will only be seen by the individual compiling the survey results in the event that a question arises regarding your particular survey feedback), and complete the survey by July 22, 2015.
