2015_Addyi Processing Incomplete Enrollments

This is a survey and not a scored knowledge assessment. The purpose is to improve our training processes. The intent is to identify your current knowledge related to processing Addyi incomplete enrollments.

  1. A prescriber is sending back an incomplete enrollment item and knowledge assessment (KA) item (adding their NPI# and correcting a missed KA question). Identify the TWO steps you would take to begin the process of entering items related to an incomplete enrollment.
    Process one of the two items (add NPI #).
    Copy the fax and then go back into the fax queue to process the other updated item (corrected KA question).
    Categorize the two items, one as NPI# and the other as KA.
    Categorize as enrollment, classify as KA.
    Attach the fax and submit.
    I'm comfortable with 1 of the 2 steps in the process, so I'm selecting this as my second step.


  1. You are working the fax queue and a pharmacy has sent in an enrollment form. You do a search and realize this pharmacy is already in the database, status of the enrollment is incomplete. It appears the pharmacy is adding their NCPDP# to complete the enrollment. The next step you would take to process this fax is?
    Select the change of information from the Add Service drop down list.
    Attach the fax and email the SCD REMS UC Help box.
    Double click the open enrollment task. If no open enrollment task, select registration fax update from the add task list.
    Complete a registration phone update.
    I'm not sure what the next step would be.


  1. You are working the fax queue and an authorized pharmacy representative has faxed in their corrected knowledge assessment (KA). The fax appears to be a corrected KA. Place the steps in the process in the correct order.

    Search for the pharmacy.  
    Double click on the relationship/affiliation for the pharmacy designee (authorized rep).  
    Click the service.  
    Click on the task to identify.  
    Add task.  
    Select assessment update fax.
