To complete your Field Placement Hours, please choose from the dates and times below.

PLEASE ALLOW **1-2** BUSINESS DAYS TO RECEIVE A CONFIRMATION VIA EMAIL AFTER YOU SUBMITTED THIS FORM.

If you have 15 HOURS to complete, please choose 3 DATES (mark 3 boxes).
If you have 30 HOURS to complete please choose 6 DATES (mark 6 boxes).
If you have 45 HOURS to complete please choose 9 DATES (mark 9 boxes).

Each day consists of five hours which will include a thorough training at the beginning of the day.

We recommend that you choose your dates as soon as possible. Students will be chosen on a first come, first served basis.
