WINTER 2016 FAU Education Students - Field Placement Hours

To complete your Field Placement Hours, please select ONE DATE from the dates and times listed below.

The time commitment is six hours and includes a thorough training at the beginning of the day.

We recommend that you choose your dates as soon as possible. Students will be chosen on a first come, first served basis.

PLEASE ALLOW **1-2** BUSINESS DAYS TO RECEIVE A CONFIRMATION VIA EMAIL AFTER YOU SUBMITTED THIS FORM.

Please bring your Rubric and Clearance Badge on the day of your visit. It is a requirement in order to volunteer with our organization.

CANCELLATION POLICY
We require at least 48 hours advance notice to cancel or reschedule a visit date, or a doctor's note for absences due to illness. We understand that situations may arise but want to remind you that your presence is necessary for our program to be successful.
