Survey for Chaplains and Social Workers
Our patients and families need access to family support services 24 hours a day. The Medicare Hospice Benefit reads, "Other covered services must be available on a 24 hour basis when reasonable and necessary to meet the needs of the patient and family". The availability of family support services enhances our quality of care by being able to meet non-medical needs on a timely basis. This survey will help us determine the best way to meet the goal of 24 hour family support services. Thank you in advance for your help!

- Would you prefer to work on an on-call basis that pays you over and above your 40 hours per week?

- Would you prefer to work on an on-call basis in which your schedule is altered to take part of the weekday off in order to manage on-call time?

- Would you prefer to work 24 hour shifts, i.e., Saturday on call, or regular hours Monday, then on-call until Tuesday am?

5pm-8 am shift?
