To assist the Social Work Practice Council in compiling a resource list, we are asking you to provide us with some information. It is important to include as much information as possible. Names, telephone numbers and address are all appreciated. Thank you.

- Is there an agency in your area that provides programs funded by the Department of Aging? Please list topics if you are able.

- Is there an agency in your territory that provides assistance with utilities? eg. CEDA
|
- Do you know of any programs that provide transportation for clients with mobility issues?

- Please list any providers you are familier with that offer daycare for clients with Dementia.
|
- Please provide the names of any emergency response systems you have made referrals to. eg. Lifeline.

- Please list any agencies you have worked with that provide Geriatric case managers.
