In November, during National Hospice Month, Midwest CareCenter will present four annual awards to staff and volunteers. They include the 2013:
1. AWARD FOR INNOVATION given to a team or department
2. SPIRIT AWARD given to a full- or part-time employee
3. WORLD-CLASS CARE AWARD given to a clinician
4. HEART OF HOSPICE AWARD given to volunteers
Nominations will be reviewed by an internal Awards Committee. These awards will be presented at the November 14 All Staff Celebration in the Glenview Atrium.

  1. 1. AWARD FOR INNOVATION is given to a DEPARTMENT OR TEAM that epitomizes the organization's mission through excellence, innovation, leadership and access. Please nominate a department or team and describe how they have fostered these four qualities in their work.


  1. 2. SPIRIT AWARD is given to an EMPLOYEE who is a stellar champion of Midwest CareCenter and was critical in the formation of an important new program, or who has helped Midwest CareCenter thrive in the midst of challenge or change. Please nominate an employee who meets this criteria and describe either the new program he/she was responsible for initiating or the way he/she helped Midwest CareCenter thrive in the midst of challenge or change.


  1. 3. WORLD-CLASS CARE AWARD is given to a clinician (physician, APN, CNA, nurse, social worker, chaplain, bereavement counselor, etc...) who promotes in an outstanding way, world-class palliative, hospice or bereavement care for their patients/clients and whose commitment to Midwest CareCenter has benefited the lives of others. Please nominate a clinician whose work continues to demonstrate a commitment to providing world-class care. Describe specific ways this clinician has made an impact on patient/client lives.
