Supplier Evaluation Mac Edwards 2018 - Produce
The purpose of this evaluation form is to rate a supplier's performance. Completion of this form will aid the Food and Nutrition Services Department in determining the quality of goods and/or services purchased for District . Your input will be used in the evaluation of future bids or proposals submitted by supplier.

- Based on your knowledge of produce used in the school program how would you rate the overall quality? Be mindful that produce is more sensitive than other products purchased. Occasional quality issues may be related to growing seasons, weather conditions, ect. outside of the vendors control.
