Supplier Evaluation Mac Edwards - Produce
The purpose of this evaluation form is to rate a supplier's performance. Completion of this form will aid the Food and Nutrition Services Department in determining the quality of goods and/or services purchased for District . Your input will be used in the evaluation of future bids or proposals submitted by supplier.

- Based on your knowledge of produce used in the school program how would you rate the overall quality? Be mindful that produce is more sensitive than other products purchased. Occasional quality issues may be related to growing seasons, weather conditions, etc. outside of the vendors control.

- Do You get the products you order?

- If the product ordered is not delivered, do you receive an acceptable substitution?

- Do you have adequate refrigeration to hold produce at the correct temperature

- How often do you return product in the course of a month due to poor quality

- How Often are you shorted an item in the course of a month.

- How many deliveries do you receive in a week
