Hey there! We understand you left before completing your program here at the ICA? It happens, and we'd like to better understand what circumstances were part of that decision for you.

A survey to former students like you is being conducted, and you're invited to participate. If you would be willing to share the circumstances under which you left the ICA program prior to completion, please complete the short 19 question survey below. Your participation will help us better identify and meet the needs of our students. Thank you for your consideration.

*This survey is being formed as part of a doctoral practicum project by Vernetta Kosalka, Banquet and Service Manager, Institute for the Culinary Arts. Participants' names will not be used, but data gathered may be used by both Metropolitan Community College and Vernetta Kosalka, M.S. as part of her doctoral work at College of Saint Mary.
