2018 - 2019 Good and Perfect Attendance

**Complete one form for each eligible employee individually.

**Please submit this form electronically by Friday, May 31, 2019 with the current eligible employee names. Indicate “None” or “No Eligible employees” if there are no employees who qualify for Good or Perfect Attendance after Question #1. All schools must complete the form even if there are no eligible employees.

**The monetary bonus for Good and Perfect Attendance is from August 15, 2018 through June 4, 2019 for Assistants and August 1, 2018 through June 5, 2019 for Managers. Good attendance applies to those individuals who missed 2 days or less of work.

**DO NOT SUBMIT ZTIM REPORT FOR ANY EMPLOYEES.
