This is the new evaluation form that we will use for trainer certification starting 9/23/2009.

  1. The instructor opened the class appropriately with an introduction and clear objective. (The instructor introduced him/herself to the class and provided a brief explanation of what the participants would be learning for the session/day…whichever is appropriate.)*
