HIPAA Authentication

To advise of the recent change to revert back to the original HIPAA Authentication requirements of 4.


A red asterisk (*) indicates required questions.

  1. Based on recent decisions within the organization, the requirements for HIPAA Authentication have been reverted back to 4 required authenticators being required.

    Effective immediately, the requirements are:
    First and Last Name
    Date of Birth
    Zip Code listed on the Main Screen
    Exception: if unable to verify zip code, the caller may verify the Plan
    Sponsor in its place
    AND one of the following:
    Rx Name
    Rx #
    Exception: if unable to verify the Rx Name or Rx #, the caller may verify one
    of the following in its place:
    Member ID
    HICN/MBI # (Medicare D)

    The HIPAA Authentication Grid (CMS-2-028920) is being updated in theSource as well as an Announcement.
